Documentation Guidelines

The following guidelines are provided to assist students in identifying the information and documentation needed in order to initiate the process for obtaining reasonable accommodations. OAS engages in the interactive process with students with disabilities and accommodation requests are reviewed on a case-by-case basis. Please note that other schools and/or testing entities may have different criteria for documentation guidelines. The student is responsible for obtaining additional documentation, if needed.

Students who received accommodations in high school are encouraged to submit previous accommodations plans and evaluations, if applicable.
