  • If you are unsure if your research needs to be reviewed by the CUHSR, submit this completed form and the CUHSR will determine if you need to complete an IRB-1 or IRB-2 Protocol Application.

  • To continue to enroll new participants past a protocol expiration date (must submit before the expiration date).

  • Any changes to an approved protocol must be approved by the CUHSR before they can be implemented (this includes changing research personnel/student RAs).
