Please use the correct checklist below as a guidance to complete the required enrollment documents. All documents can be e-mailed to martinezmar@easternct.edu or faxed to 860-465-5277.
All families must submit income verification. Failure to do so may result in defaulting to the highest tuition rate.

Household Income Verification (copy of tax return, paystubs 2 if biweekly or 4 if weekly)
Proof of Residency
Physical and Immunization Record (If asthma or allergy diagnosis, Authorization of Medication must be complete and an Asthma Care Plan and/or Allergy Care Plan.)

W-9 (please hand deliver on first day)

Welcome back!
We are happy to know that you will be continuing to attend the Child and Family Development Resource Center this coming School Year! As we begin the transition into the new school year, we ask you to review in the Renewal Packet. These documents must be renewed no later than Friday, May 17th.
In addition, you will be required to complete a new tuition contract for the 2024-2025 school year effective August 26th. We will need current (within 3 months) proof of household income in order to determine your tuition. Please provide current proof of income (copy of tax return, paystubs 2 if biweekly or 4 if weekly) no later than May 17th. Without current income verification, you may risk defaulting to the highest rate on the fee scale.
