Background Check Process

School Districts
  • Andover Background Check Procedures: 

    Upon placement confirmation,
    you will need to complete the background check for Andover Elementary School at your earliest convenience. Check in with the Main Office to inquire about any newly required procedures. Otherwise, student teachers need to be fingerprinted at EASTCONN (unless they have proof of a background check from the university).

  • Please see the Cromwell Public School District’s university intern/student teacher requirements below:

    1.       Meet with your assigned prospective cooperating teacher to determine if a placement will be accepted.

    2.       Once the cooperating teacher agrees to accept the placement, sign the confidentiality paperwork provided by your cooperating teacher (see attachment).

    3.       Bring a copy of the signed confidentiality paperwork to Central Office as verification that you’ve received placement within district.

    4.       At Central Office, request our university intern packet that explains the fingerprinting process necessary to work with Cromwell Public Schools’ students.

    5.       Follow each building’s entry protocol by entering through the main office, signing in, and receiving a visitor’s badge (if applicable, your cooperating teacher will advise).

    Please make sure the above steps are followed as Cromwell Public Schools strives to protect the safety and well-being of our students.

  • East Haddam Background Check Procedures: 

    Contact cindy.hughson@easthaddamschools.org in the central office to begin your background check.

  • East Hampton Background Check Procedures: 

    We have attached a packet for student teacher’s to complete.  Also, we will provide a customized letter for the applicant to give to our Police to get fingerprints. Currently, we are not charging student teachers for those processing fees. 

    These documents will need to be completed prior to being placed in a school.  Also the student would go thru our “Raptor” driver licensing scan in the school’s office prior to first time admittance.

    Faculty and Staff Fingerprinting 

    Fingerprint appointments are between the hours of:

    S/O Collins 9:00 AM until 12:00 PM 
    S/O Crandall 4:00 PM until 5:00 PM 
    Monday thru Friday in the High School Main Office 

    1. Print and complete the Fingerprinting Information Sheet using the instruction provided. Return at least 1 day before your appointment to the High School Main Office. 
    2. Appointment can be made at the Central Office for employees and at the Elementary school for volunteers. 
    3. For employees, please bring a CERTIFIED BANK CHECK or MONEY ORDER made out to "Treasurer, State of Connecticut" for $12.00 with you during your appointment. (PERSONAL CHECKS ARE NOT ACCEPTED)

  • Hebron Background Check Procedures: 

    Please contact Jan Shorts, Administrative Assistant, at jan.shorts@rhamschools.org to begin the background check process. 

  • Lebanon Background Check Contact Information: 

    Please contact Central Office to get the background check started as soon as possible: Superintendent Administrative Assistant Chelsea Williams at Chelsea.williams@lebanonct.org.

  • Lisbon Background Check Process: We will need fingerprinting done (through EASTCONN) and the DCF Background Check. Instructions for fingerprinting and the DCF form can be picked up here at Lisbon Central School.  

  • Manchester Background Check Procedures: 

    Eastern students placed in the Manchester school district have options for fingerprint/background check process as explained in this attached fingerprinting grid 

    To clarify the information found on the above attached grid, you have 2 options: 

    Option # 1: CREC/EASTCONN – student teachers (and other clinical candidates) have to make an appointment, and have a CHRI form signed by Manchester to go.  The only time a student will need a CHRI form is if they go to CREC/EASTCONN for fingerprinting. Student teachers fall under the Federal Statute AWA Volunteer on the CHRI form. Please complete the top, send to Manchester district facilitator, Heather Gates at heathere@mpspride.org for signature, and she will send it back to you once signed.  

    Option #2: Fingerprint Card - which can be done at either a biometrics ($25) location or the Police Barracks ($15) in Middletown. After they have the card, they need to bring the card, AND two separate money orders both made out to "Treasurer - State of CT" (the state does NOT accept personal checks) to our Central Office (45 North School St). The money orders should be for $11.25. Students who go this route do NOT need a CHRI form. 

    Questions regarding prints should NOT go to the Manchester HR Dept unless prints have been submitted already. If you still have question(s), you can email district facilitator, Heather Gates at heathere@mpspride.org. 

  • All interns and student teachers for the Mansfield Public Schools are required to be fingerprinted through EASTCONN 30 days prior to starting your placement.

    EASTCONN fingerprinting brochure with information regarding the process—highlights below:
    An appointment MUST be made in advance—contact 860.455.0707 for an appointment
    There is a $130 fee due at the time of printing.
    You MUST bring the Fingerprint From: Criminal History Record Information Request Form (below)
    Bring one (1) form of ID with you.

    Fingerprint Form: “Criminal History Record Information Requisition Form” (CHRI Form).
    Complete pink highlighted sections. MUST bring this form to your appointment at EASTCONN.

    Print Waiver:

    Please review and complete. Completed form MUST be returned to me along with a copy of the letter EASTCONN provides upon fingerprinting as proof you’ve been fingerprinted via mail, fax or email to:

  • Marlborough Background Check Contact Information: 

    Please check out Marlborough’s school website to begin familiarizing yourself, and stop in at the Main Office or contact them with your interview date so they can advise you regarding the background check procedures:

  • Montville Background Check Procedures:

    For student teaching we require that you complete background checks in the form of  a DCF background check as well as fingerprinting. The fees for fingerprinting are to be made in the form of 2 checks or money orders, one should be for $75.00, and the other for $13.25, both made out to “Treasurer State of CT”. These fees get sent off with your fingerprint card to be processed by the State Police.  I have attached the DCF Form that must be completely filled out and returned to me at the time of fingerprinting. Please note the that the form asks for anyone living in your household, aged 18 years of age or older to sign off on the form.

     

    In addition to the background checks, It is mandatory that you complete 2 online training sessions, one for Mandated Reporter Training and the other for - Blood borne Pathogens Training. The training does not work on Chrome, so please use Internet Explorer.

    Norwich Public Schools Background Check Procedures: 

    Attached are the 2 forms that must be completed. Please note that a money order in the amount of $65.00 must be made out to Norwich Public schools. The forms and money order can be dropped at our Central Office at 90 Town St Norwich, Ct 06360. The office is open until 5 PM. Should you have any questions, please do not hesitate to contact Natalie Rashleigh at nrashleigh@norwichpublicschools.org. 

  • Norwich Free Academy High School:  Stop in the main office at NFA when you are there for your interview to start the background check process. Please bring your ID.

  • Plainfield Public Schools Background Check Procedures: 

    The students should call Becki Miller @ 860-564–6413 or email at millerb@plainfieldschools.org to make an appointment for a background check. 

  • Contact Information for RHAM Background Check Procedures: 

    Connect with Kathy Mund at kathy.mund@rhamschools.org in our Central Office to fill out necessary paperwork. 

  • Somers Background Check Procedures: 

    Our policy for student teachers is on our website under employment.  Here's the link:

    Students should contact Joan Jaquith, Secretary, to get the paperwork they need.

     

  • Stafford Background Check Procedures: 

    The students will need to complete the district application for student teaching placement.  All student teachers are required to complete background screening (DCF, Fingerprints, and Sex Offender Registry) prior to working in our schools.  Once the student completes an interview with the cooperating teacher, myself, Steve Autieri, and the building principal, we will forward the student to the Human Resource Specialist, Lori Dobson to complete these steps.

  • Waterford Background Check Procedures: 

    You will need to complete, the Waterford district background check. See attached link: http://www.waterford.k12.mi.us/pages/50959/human-resources 

  • Please see the attached updated forms for students being placed in Windham. All documents will need to be completed and turned in to our offices before beginning any placement. Fingerprinting is only required if the Eastern student will be unsupervised with the children at any point of time.   • Windsor Background Check Procedures: 

    I would like to make you aware of our new fingerprinting/internship procedures in Windsor. 

    The procedures can be accessed here at our new Induction & TEAM Support webpage and our forms are accessible at our EPP Partnership page.  Once any candidates are interviewed and plan to be in Windsor, they need to begin the paperwork process with Stacy Jenkins-Murphy in HR immediately. 

  • Background Check Procedures for Windsor Locks Public Schools 

    Please contact Ann Marie Charette, in Human Resources, at 860-292-5744, to set up a time to pick up the required forms, criminal background check, DCF and fingerprinting. We ask that you bring your driver’s license with you for identification purposes. The office is located at 58 South Elm Street in Windsor Locks.
