Important message from Athletic Training

As of Fall 2017, part-time undergraduate students and part-time graduate students will not be eligible to participate in Club Sports

Instructions to complete medical clearance for the 2019-2020 academic year

I wanted to take a moment to communicate with you regarding how COVID-19 will impact the medical clearance process in the fall. Knowing the impact on the medical community, we ask that you communicate the following to your student-athletes:

  1. Schedule your PPE examination now, but be patient, you may not get an appointment until later this summer. Bring the university PPE form with you to the appointment, that is the only form that will be accepted. Do not wait and rely on Student Health Services to complete your PPE, that may no longer be an easily accessible option.
  2. Returners – update SportsWare account, complete consent form and complete medical history questionnaire ASAP. Medical history questionnaire should have a description for every YES answer. If they do this ahead of time, it saves me from sending a LOT OF EMAILS.
  3. Student-athletes diagnosed with COVID-19 may face additional testing/screening. We are working on developing this return to play policy as research and evidence are presented to us. If you know of any that have been impacted by a diagnosis, please let me know.
  4. All students will have some degree of education, screening and prevention that are required. Processes are currently being developed. There may be additional processes required by intercollegiate and club athletes. We’ll keep you up to date on that as more information becomes available.

I’ve added the requirements in a .pdf attached, one for newcomers, one for returners. I’ve also added the information on how newcomers access their SportWare EMR account. They cannot access until we create their account, so get rosters to me when you can!

    Dear Student-Athlete, 
    Prior to participating on a team at Eastern Connecticut State University, student-athletes must provide the Athletic Department with medical clearance information. To expedite this process, Eastern Connecticut State University uses an online data entry system.

    If you are a returning member of a team, recruit or transfer participating for the first time, please follow the directions below to set up a SportsWare Online account.

    1. Go to www.swol123.net
    2. Enter your Eastern Email Address and click the Reset Password button. (New athletes, please enter the email address that you provided to your coach.) You will receive an e-mail with the Subject “SportsWare OnLine Password Request”. Open the e-mail and click on the link to reset your password. Enter your e-mail address, new password and click the Save button.

    2. Enter your Email Address and click the Login button. At the top of the page is the Menu Bar.
      1. My Info: Update your address, emergency contact and insurance information.
      2. Med History: Complete a Medical History questionnaire.
      3. Forms: View/complete required paperwork. Please refer to the Medical Clearance Instructions for Student Athletes below for all requirements.
