Student Conduct Resources for Students

In an effort to help guide the success of completing student conduct imposed sanctions, the Office of Student Conduct has published numerous documents, forms, and informational packets for students. These published documents can either be picked up in the Student Conduct Office in Wood Support Services, Room 254, or by downloading the forms below.

Academic Integrity Workshop

Academic integrity is an essential component to academic success and ethics. Students who want to know more about the Academic Misconduct Policy are encouraged to visit the plagiarism web page and review the excellent resources there. In addition, students may take a free Academic Integrity Workshop by contacting the Student Conduct Office.

Student Conduct Forms/Documents

Community Restitution Verification Form: Used for verification that a student has completed their community restitution hours at a student conduct sanctioned/approved agency. The form requires the signature of both the student and volunteer coordinator of the agency where the restitution hours were completed.

Note: The community restitution requirement due date is determined by the Director of Student Conduct.

University Student Conduct System Application for Review: Please fill out this form if you would like the Office of Student Conduct to review a decision made during the Student Conduct process. Please note: this form must be filled out no later than three University Calendar days after the Notice of Decision Letter is delivered.
