APPLICATION PROCESS

1. Application form for Summer Study Program
2. Statement of Assumption of Risk, with your signature notarized. ECSU students, see Mary Ann Coughlin, Notary Public, Shafer Hall.
3. Health Insurance Form.
4. A payment of no less than $500 with the application, which will confirm you registration in the program, flight reservations, and accomodations in Spain. You may pay the total cost of the program as follows:

a) $2, 900 with application,
b) $500 with application,
    $1,200 by April 1, 2006
    $1,200 by May 1, 2006

Make you checks/money orders payable to "Summer Study in Spain." You can also make payments with a credit card - Visa, Master Card, or other at any Fleet Bank Branch office. The account name is: "Summer Study in Spain". If you use this form of payment, notify us immediately and send us a copy of the bank receipt.

Variable amounts of financial aid may be available to you through the institution you are affiliated with. ECSU students are advised to consult with Richard Savage,  Director of Financial Aid (465-4434).

5. A copy of your passport's front pages. These pages show your photo, signature, dates of issue and expiration, passport number, name, and date of birth. Mail/bring it to us as soon possible. If you still don't have a passport, you must obtain one immediately. Applications are available at any post office, and the process takes 3 - 4 weeks.

6. If you will not travel with the group from NY to Madrid and back on the scheduled dates - May 27 and June 26, 2006, we need a written statement of your traveling plans indicating departure and return dates.

Individual arrangements can be made for traveling on dates of your choice, but if you make the arrangements through our program, the departure and destination point (NY-Kennedy, and Madrid-Barajas) can not be changed.

It is essential that the application process is completed as soon as possible so that we know the exact number of participants well ahead, have plenty of time to secure the best accomodations available, and can make detailed plans for transportation to, and in Spain. I urge you to submit all the required items promptly and start making preparations for the trip. We will communicate with you further between now and the take off time to ensure that you are ready for the experience. Meantime, we welcome you to our program and look forward to sharing with you a productive and fun time in Spain.
