General Instructions and FYP Committee Guidelines
This form should be printed and read thoroughly before proceeding to Parts 1, 2, and 3.
FYI 100 First-Year Introduction Proposal Forms
Parts 1 and 2 are electronic surveys. You will not be able to save either of these and return to finish at a later time. You must complete these surveys once you begin or cancel and start over. We suggest that you first go to each survey and print a blank copy using your computer's print function. This will allow you to plan your responses before beginning and completing each survey online.
Part 1 - FYI 100 Approval Form/Survey
This survey form will allow interested faculty members to inform the FYP Office of their intention to offer FYI 100 during the Fall 2012 semester. Additionally, faculty will be asked to provide a course title, description and some other information. After submitting Part 1 to the FYP Office, Part 2 should be completed as soon as possible.
Part 2 - FYI Approval Form/Survey
This survey asks faculty members to provide written explanations on how they will achieve the various goals and objectives of FYI 100. After submitting Part 2 to the FYP Office, Part 3 should be completed as soon as possible.
Part 3 - FYI Approval Form/Syllabus
This is an instruction sheet for creating your syllabus. Part 3 should be completed as soon as possible. The FYP Committee will not be able to evaluate your proposal until Parts, 1, 2, and 3 have been submitted.
Written Sample Syllabi
These syllabi are provided to help guide you when preparing your own syllabus. Each syllabus shows a different approach to creating your FYI 100 course. The creation of a new FYI 100 course is illustrated in syllabus 1, converting an existing LAP 130 course into FYI 100 is illustrated in syllabus 2 and creating a new FYI 100 course to be taken by a student in a specific major is illustrated in syllabus 3.
