Monthly expense and resource form.
Child Support Paid Worksheet

Form to report child support paid by you or a member(s) of your household.
Citizenship Certification AffidavitForm for the collection of Department of Homeland Security of other U.S. citizenship/nationality documents from students unable to present their documents in person
Confirmation of Dependency Status

Used to clarify the dependency status of a student.
Consortium AgreementUsed in the event the student may also be taking classes and receiving aid at another college.
Dependent Non-Filer

Dependent students or their parents who did not file taxes may need complete this form.
Dependency Override Appeal Form

Used when a dependent student requests consideration as an independent student.
Dependent Verification Worksheet

Used by dependent students selected for verification.
Direct Lending Entrance CounselingAll Federal Direct Lending Stafford Loan Borrowers must complete this form prior to the release of Federal Loan Funds. Note: This website currently only works correctly with Internet Eplorer or Firefox.
Direct Lending Exit CounselingAll Federal Direct Loan Borrowers who are graduating must complete this EXIT Counseling form prior to leaving the University. Please only use this form for Direct Loans.
Direct Lending Master Promissory NoteAll Federal Direct Loan and PLUS Loan Borrowers must complete this form prior to the disbursement of loans. Note: This website currently only works correctly with Internet Explorer or Firefox.
Estimated Income Form

Used to estimate the changes in taxable and untaxable income from 2015 to 2016
Family Clarification

Used to clarify family size and number in college.
Food Stamps/SNAP Verification Worksheet

Used to verify that you or a member(s) of your household received Food Stamps or participated in the Supplemental Nutrition Assistance Program (SNAP) during the past calendar year.
Identity and Statement of Educational Purpose

Used in verifying a student’s identity. Note: Where possible, please present this form and a valid government-issued photo identification (ID), such as a driver’s license, other state-issued ID, or passport, in person at the Office of Financial Aid. If the student is unable to appear in person they may use this form to complete the requirement.
Independent Non-Filer

Independent student non-tax filers may need to complete this form.
Independent Verification Worksheet

Used by independent students selected for verification.
Information Release

Authorization form to release financial records of the student to others.
IRS Form 4506-TUsed by students or parents to send a request to the IRS for either a Transcript of their Tax Return or letter verifying non-filing status.
Satisfactory Academic Progress Appeal Form

Used by financial aid recipients who fail to meet the GPA and/or credits completed requirements for Satisfactory Academic Progress (SAP).
Satisfactory Academic Progress PolicyPolicy statement on Satisfactory Academic Progress (SAP) requirements to receive financial aid.
Short Term Loan

Application for Short Term Loan against expected financial aid refund.
Special Circumstances Application

Application for consideration of re-evaluation of financial aid based on special family financial circumstances that may have adversely affected the family income.
Summer School Information Sheet

All students seeking summer financial aid must complete the Summer School 2017 Information Sheet which can be downloaded from our website.
Perkins Loan Borrower rights/responsibility

Perkins Loan Borrowers must complete this form and return it to the Office of Financial Aid.
Perkins Loan Exit InterviewPerkins Loan borrowers who are graduating must complete this form. This link will bring you to University Accounting Service (UAS), once the form is completed we will download the forms to complete the exit process.
Verification of Family Enrollment
Used to verify family members enrolled in college.
Note: Please contact the Registrar’s Office at 465-5224 regarding any questions about residency reclassification.
