Faculty and Staff Judicial Resources

The  Office of Judicial Affairs has developed numerous resources to aid University faculty, staff, and administrators in filing judicial complaints against students for violating a University policy.  These resources are available in the Judicial Affairs Office in Wood Support Services, Room 246 or can be downloaded below.

Faculty and Staff Judicial Forms/Documents:

Student Notice of Charges Form: A legal document that is required to be filled out by faculty or staff person filing the formal judicial charge against a student.  After completion the form should be sent to the student directly to inform them that charges are being filled against them.  (Student Notice of Charges)

University Judicial Charges: A spreadsheet document that contains the identification letters, numbers and details of the different charges that can be filled on a student for violating University rules and regulations as indicated in the Student Handbook.   (Judicial Charges)

"How To" Guide to Filing Judicial Charges: A document containing detailed directions and information about how to file a formal judicial charge against a student.  Contains information about how an incident report should be filled out, to how to file a judicial complaint, and more.  ("How To" Guide to Filing Judicial Charges)

Incident Report Form: A legal document that is required to be filled out by the faculty or staff person filing the formal judicial charge against a student.  The Complainant (person filing the charges ) will provide a detailed description of the incident for which the student is being judicially charged.  Directions on how to fill out this form are contained in the "How To" Guide to Filing Judicial Charges. 

(Incident Report Form)

Campus Complaint Form: A legal document that is required to be filled out by the faculty or staff person filing the formal judicial charge against a student.  This document serves as the formal request for judicial charges to be filed, and can be seen by the student upon request.  It is brief and describes the basic facts of the incident.  Directions on how to fill out this form are contained in the "How To" Guide to Filing Judicial Charges (Campus Complaint Form)
