Health Entrance Requirements
Immunizations (Required for all full-time and part-time students).
Connecticut State law requirements regarding measles (rubeola) and German measles (rubella) are strictly enforced. ALL full-time and part-time students born after 1956 must provide documented proof of two doses of measles (rubeola) vaccine and one dose of rubella.  Students who do not submit these records will NOT be allowed to register.  Meningitis vaccination is required only for students who live in on-campus housing however we recommend it for all incoming students. Additional immunizations are required and/or recommended for full-time students. See the "required form" link below for details.
Full Time Students
Physical Examination

All entering students must have a complete physical examination within one year prior to registration. This is not required for graduate students.

Download the required form for undergraduate students.

Tuberculosis Screening (full-time students)
The Connecticut State University (CSU) System requires all incoming students, regardless of age, to complete a Tuberculosis Risk Assessment to provide documentation of TB risk.  This risk assessment (included with the forms on this page) should be reviewed with your health care provider.
Transfer Students

Transfer students must meet the same requirements as other Eastern students.  However, if you have already gone through this process for your previous institution, in most cases, you DO NOT have to repeat it for us.  Instead, we ask that you have the information you submitted to your previous school sent to us.  We will review it and if any of the items we require are missing, we will contact you.  You DO NOT have to get another physical examination if you had one for your previous school.  The only exception would be if your health status has significantly changed due to illness or injury since your last exam.  Full time students still need to complete the first 3 pages of the CSU Confidential Health Form which should take only about 5-10 minutes.

The Health Examination Report and immunization records are maintained on file at the Health Service for a period of 7 years. They are CONFIDENTIAL health records. They will NOT be released without written consent of the student.

Part Time Students and Graduate Students

Part Time and Graduate Students must submit proof of measles (rubeola) and German measles (rubella) immunization as outlined above along with completion of the tuberculosis risk assessment . A physical examination is NOT required for part-time or graduate students.

Download the required form for Part - Time Students.

Any student failing to submit a completed Health Examination Report and/or proof of required immunizations WILL NOT be allowed to register!
