Interview Form of Educators Using Computers in Education for EDU 360

Interview educators who use computers for curriculum development and for instruction.  You may submit more that two of these surveys.

Please do NOT interview yourself unless you are teaching at least part-time.

Please complete the following steps to email this form to me:

1) Highlight the survey form from Interviewer to END of EDU 360 Survey.

2) Under Edit, Copy.

3) Create a new email message by clicking on Ms. Sellers for EDU 360 and Dr. Stoloff  for EDU 553 .

4) While still in your email,  Under Edit, PASTE the copied form into the text field of your email message, the space where you would write your message.

5) Complete the form by typing your answers under each of the questions.

6) Send the completed form to the appropriate instructor - Ms. Sellers for EDU 360 and Dr. Stoloff  for EDU 553 .

In Netscape,
Under Edit, chose Preference.
Click on Mail and News Groups.

Type in Your Name and Email Address.  Click on OK.
